Twins usually feel looser and more responsive. Keels usually add more down-the-line drive and hold, especially on classic fish shapes.
A twin+1 adds a small stabiliser fin between the twins. It keeps much of the twin-fin speed while adding extra control.
They can work, but most surfers prefer the extra control of a thruster, quad or twin+1 when the waves are solid, steep or choppy.
